Digital Revolution Co-op Program Form

By using this online form, a South Dakota visitor industry-related business or event may submit a Digital Revolution co-op program registration.

We ask that all fields be completed; although, email is the only mandatory field. We also ask that you indicate the region in which your business is located, whether you are registering for the $250 option or the $500 option, and how you are submitting your payment.

Registration is not complete until payment is received.

Registrations are accepted online or via a downloadable form (PDF 25 KB). It may be faxed to Brad Lemmel's attention at 605-773-5977.

NOTE: Department of Tourism staff will review and may decline partner registrations as deemed appropriate. If you are uncertain as to whether or not you qualify, please contact the Department of Tourism prior to submitting your registration and payment.
